CALIFORNIA CLINICAL PHARMACIST
Job Description:
Our client is seeking a skilled and experienced Clinical Pharmacist to work
under the direction of the Pharmacy Coordinator. The chosen candidate
and/or Clinical Pharmacist is responsible for optimal therapeutic medication
regimens for acute care, home care, critical care, oncology, transitional care
and ambulatory care for adult, pediatric and geriatric patients through
reviewing, assessing and intervening in patient medication therapy, supervising
the order entry and dispensing of medication orders, providing drug information
to health care professional and patients and overseeing the application of
pharmacy law with hospital/pharmacy policies and procedures on a day-to-day
basis. The chosen candidate will monitor, evaluate and assess therapeutic
regimens and target medications for acute care, critical care, oncology,
transitional care and ambulatory care patients based on the needs of pediatric,
adult and geriatric patients. Identify drug-induced problems and intervene
with physician and other professional staff and provide timely and accurate
pharmacological and therapeutic consultation to Physicians and other
professional staff. . The Clinical Pharmacist will supervise
technicians on a day-to-day basis to ensure the safe and appropriate dispensing
of medications and supervise and participate in the processing and dispensing of
medication orders according to federal and state regulations and hospital
policies and procedures. The Clinical Pharmacist will supervises the
procurement, control, preparation, order entry and dispensing of medications and
maintain accurate and secure narcotic and hypnotic inventory and record
maintenance.
Requirements:
Qualified candidates must possess a Current State of California Board of
Pharmacy License and a Doctor of Pharmacy Degree or equivalent skill set in
clinical activities as determined by the Pharmacy Coordinator. Qualified
candidates must possess a Minimum 1 year experience in acute care hospital (new
residency graduate qualifies). Candidates must also have successfully completed
the ASHP Accredited Residency in Clinical Pharmacy preferred. You must possess
the ability to manage time productively for daily activities and projects. In
addition, you must also possess the ability to maintain effective communication
and interpersonal skills.
